While employers have the benefit of management information tools like P&L accounts, balance sheets, production and maintenance reports and sales pipelines, there are no equivalent tools to help manage employee health costs – until now. Elements provides each client with knowledge tools and aggregated predictive data that provides a health & well-being fingerprint of their population. By linking business drivers to the Employee Health & Lifestyle Management Program, Elements optimizes individual and corporate performance. Elements analytic tools allows management real-time insight into their current health plans providing accurate, actionable information to manage their health care expenses as they fiscally manage every other aspect of their business. We call this Sustainable Insurance. We are the only consulting firm who can truly state that we – Put Health into Health Insurance.

Our program is unique in that it is not a corporate wellness program or a claims analysis program but rather an integrated health solution customized for each client based on their needs and culture.

To identify those needs along with the culture we leverage our proprietary Health and Lifestyle Assessment program along with our Health Plan Analytics Software to build an employer profile.

We use this profile as a working document to help design the appropriate health benefits program integrating health insurance with a pro-active Health and Lifestyle Management Solution. Each employee is provided access to their own personal health assessment profile along with a health coach and the Five Elements of Health proprietary Lifestyle Management Program (see below for additional details)

Each of our clients are provided with ongoing real-time Health Plan Analytics integrating the employee aggregate health assessment data with the health plans medical and Rx claims and utilization data in order to gain a comprehensive and intelligent view of what is driving one of the largest line items effecting each employers bottom line.

PERSONAL HEALTH PROFILE

Our assessments ask a series of simple questions about your health history. You can tell us about your symptoms, diagnoses, medications, diet, lifestyle habits, and laboratory test results. We use each piece of information to build a unique and personalized profile. This profile is used to calculate a series of health risks, as well as to generate a personal guidance report.
Your employees will be given highly accurate predictions of important health risks, plus a prioritized list of simple steps to reduce the chance of getting seriously ill. With roughly 400 potential recommendations, and a nearly infinite number of potential assessment patterns, no two reports are exactly alike.
Any system can identify the high expenditure employees based on current symptoms, but we can help you figure out who will cost the most money in the future, and intervene before problems arise.
Your employees can set up reminders to track progress. You will be able to track which employees are regularly using this system to provide incentives and rewards.

Aggregate Health Assessment Data

You will receive an aggregate report of your employees' health risks and behavior patterns. You can use this report to prioritize your preventive services. (We transfer all personally identifiable data via secure connection. We will never share the sensitive health data or email address of your employees)

    Drill-Down

    Elements uses state-of-the-art online analytical processing known as OLAP. This powerful querying engine formulates your data into robust "cubes" that allow it to be viewed in limitless variations. Elements Drill-Down can answer nearly any question you have about claims experience, including:

    • Why are my office visits so high? Who's going to the doctor? Why are they going? Are they using in-network or out-of-network providers?
    • Why are my members using the emergency room so frequently? Who's using the ER the most — employees, spouses, or dependents? Are they using the ER appropriately?
